The Youth Connection Career Academies (TYCCA) Program Facilitator calls for the coordination of workforce development, case management services, and program activities. These services and activities include, but are not limited to, life coaching and career advisement, employment and life skills training, employment placement assistance, educational supports, referrals, and supportive services to aid youth clients in achieving their employment and educational goals. The facilitator will help with intake, enrollment, and orientation. The TYCCA Facilitator handles managing case flow by assessing client needs, assigning caseloads, and monitoring and evaluating services, action plans, and progress. This position works in coordination with other TYCCA team members and closely with community partners to support their youth and overall programming to meet and exceed contract requirements.

What You’ll Do:
  • Assist Intake & Resource Specialist with managing incoming referrals and enrollment processes. 
  • Serve as the Lead Youth Advisor that provides support on boarding training to new youth advisors/case managers. 
  • Process intake and conduct youth client assessments to determine eligibility, suitability, and needs to develop individual, youth-driven action plans and service strategies. 
  • Assist with managing case workflow by coordinating receipt of vital documents, assessments, services, and all paperwork per the contract terms (i.e., Individual Service Strategies, Employment, Education, and CASAS Testing, etc.) 
  • Ensure no gaps in service and assist with assigning cases based on client needs and existing caseloads.
  • Monitor cases and coordinate with TYCCA Staff to ensure individualized client services program facilitation
  • Provide case management to assigned youth clients; not to exceed no more than a one-third of active WIOA youth and complete program reports as needed.
  • Ensure all client files are complete with detailed, monthly case notes and documentation in OSMIS to manage appropriate follow-up and reporting purposes. 
  • Work with TYCCA staff to assess client skills and interests to provide customized support such as job placement assistance, addressing employment barriers, and work readiness training. 
  • Assist with implementation of client educational activities, coordinate talent tours and focus trips, and assist with site monitoring of summer job placements and attend various field trips, group outings, etc. 
  • Provide orientation to youth on the services and eligibility requirements 
  • Assist clients with determining eligibility, registering, and accessing community and social service needs. Provide transportation as necessary for clients (either directly or indirectly) to ensure clients are able to take advantage of the services needed and all required program activities.
  • Maintain contact with assigned youth at least once per month, making additional contact as needed to ensure appropriate follow-up and reporting.
  • Ensure youth participate in life skills and other training and workshops to develop the skills necessary to achieve economic independence and effective life skill management. 
  • Work with TYCCA Staff to assess client skills and interests to provide career planning, post-secondary education opportunities, and job placement assistance. 
  • Assist with monitoring of summer job placements and participate in field trips, group outings, etc. 
  • Conduct worksite visits as required for program compliance.
  • Provide 12-month follow-up support to assist clients as needed in maintaining self-sufficiency after completing career academies programs and services. 
  • Develop and maintain contact with organizations and educational institutions, making TYCCA presentations for the purpose of outreach, recruitment, and securing opportunities for TYCCA youth. 
  • Maintain accurate, up to date client files.
  • Meet all contract requirements and other duties as assigned.
 
What You Bring:
Required:
  • A bachelor's degree in psychology, counseling, social work, human services, or a related field. 
  • Two years’ experience working with youth and young adults.
  • Background checks and clearances as required. 
  • Have a valid driver’s license, reliable transportation and auto insurance.
  • Must have case management experience with youth ages 14-24. 
  • Knowledge of MS Office, including Word, Excel, Access, PowerPoint, and Outlook.
  • Work primarily forty (40) weekday hours, with occasional evenings and weekends. 
  • Travel within the Metro Detroit area.
